• Existing patients can request appointment through portal choosing time most convenient time for them

All NEW patients to HLS Endocrinology Physician Care need to complete clinic forms and questionnaires assigned to new patients. Dr. Duchon & the team will review all information provided BEFORE the consultation what helps with formulating a management plan. If you have medical records to share please do so via the secure portal.

Failure to submit forms in time may result in cancellation of your appointment.
